The time from when you start having menopausal symptoms until your periods stop can last as little as one to two years, but for most women it lasts an average of five years. The symptoms of menopause often intensify during the two years immediately before menopause. This is when the drop in ovarian hormone secretion speeds up.

Your first clues that you are entering perimenopause:

Hot flashes (HF)

Hot flashes and sweats are the most common and bothersome complaint of women approaching menopause. They frequently begin up to five years before periods stop and can last for another five years after. Nearly 80% of women suffer hot flashes (HF) to some degree during the transition into menopause. 30% of women report HF to be severe enough to significantly disrupt their quality of life. In 10% of women HF are experienced up to ten years after their menstrual periods stop. HF affect 75% of women over age 50.

At the onset, hot flashes often occur just before or during menstrual periods. They can range from mild, to severe and debilitating. They are often described as a sudden wave of heat moving through the body, from the chest to the neck, face and scalp. The skin of the neck and face can flush red. HF can also be experienced across the breasts, below the breasts, or as a surge of heat over the entire body. The wave of heat is generally followed sweating. This sweating can range from light perspiration to heavy sweating and chills. Intense hot flashes may also be associated with a racing heart, tingling in the hands, and nausea.

Night Sweats

Night sweats are an extension of hot flashes (HF), experienced at night while trying to sleep. At the culmination of a HF one begins to perspire. At night time this is experienced as sweating which can be so severe that one wakes up drenched in perspiration. These night time HF tend to disrupt sleep. Problems with sleeping is another symptom of perimenopause and menopause, that may be a direct result of night sweats, sleep problems, fatigue and problems with memory. All of these complaints are thought to be associated with declines in the ovarian hormones estrogen and progesterone. The severity of these complaints seems to be in direct proportion to the severity and frequency of hot flashes.

Our current understanding is that menopause sweats and hot flashes are caused by dropping levels of estrogen. Unfortunately hormone replacement therapy--replacing the estrogen that the ovaries are not making--has potentially severe side effects. The two most dangerous are breast cancer and heart disease.

Molecules resembling estrogen in structure and function are also found in nature. These are called phytoestrogens (phyto=plant). That is to say these molecules are found in plants. The patient should have hormone diagnostic blood work or saliva tests to determine if there are any significant problems. These may be addressed with bio-identical hormones for long-term use. Testosterone, androgen, progesterone, estrogen, melatonin, thyroid, DHEA, and pregnelone levels may be impaired and need to be supplemented. This may a key to solving migraines and will also be useful in your anti-aging strategy to stay youthful. These hormone may partially be treated with animal glandular hormone supplements. If the person is middle aged, these supplements may be successfully used for many years. This may also protect your brain from memory loss, depression, and dementia. Other strategies to balance hormones include Chinese patent herbal formulas like Xio Yao Wan or Jia Wei Xiao Yao Wan. These formulas can help women in menopause or peri-menopause.

Migraine headaches may occur due to food or environmental allergens acting as triggers and setting off the migraine. The allergens need to be identified and avoided through the G.A.R.D. The immune system calms down when there is not frequent exposure to the allergen. Migraines may also be a simple matter or neurotransmitter imbalances. The serotonin neurotransmitter is out of balance and can be addressed with amino acid therapy. The amino acids that may be useful in relieving migraines include 5-HTP, SAME, theanine, and tryptophan. These amino acids need to be taken with appropriate cofactors and enzymes for optimal results. Always remember to include a food based multiple vitamin on a daily basis. The amino acids will be taken in therapeutic doses along with vitamins and minerals. Folic acid and magnesium are vitamins that are frequently deficient in people experiencing migraines. It is also key to include Omega 3 and 6 essential fatty acids on a daily basis.

National Patient Safety Goals 2013: Universal Protocol


In 2002, the National Patient Safety Goals (NPSGs) were formulated by the Joint Commission and became effective in January 2003. These goals were set up to help covered healthcare organizations identify and address specific issues relating to safety of patients.

Many times the safety of patients in healthcare organizations may be compromised. The purpose of NPSGs 2013 is to identify specific areas where this could happen and then address these issues by formulating goals to implement measures throughout the organization. Specific attention is given to properly identify the patient before administering medicines and performing procedures. Other areas of focus are infection prevention and good communication.

These goals are updates regularly to keep up with new developments. The updates are the responsibility of the Patient Safety Advisory Group. This group is comprised of skilled professionals working in different healthcare facilities and involved with patient safety matters. They include doctors, nurses, clinical engineers, pharmacists, and so on. Having such people on the advisory board enables accurate identification of risks and results in active contribution by all the healthcare organizations. The Patient Safety Advisory Group works in collaboration with the Joint Commission to identify new threats to patient safety. Once identified, they develop plans to address these threats. After carefully considering all the factors the Joint Commission uses the most effective approach to address the issues in order of priority.

A new addition to the NPSGs 2013 is the Universal Protocol for Preventing Wrong Person, Wrong Site, and Wrong Procedure Surgery. This universal protocol is related to all invasive procedures, whether surgical or non-surgical. It has been previously established that the most risky procedures are those that involve deep sedation or general anesthesia. Healthcare facilities can promote safety by accurately identifying the patient, the procedure, and the site of procedure.

These are the four principles of the Universal Protocol:

• Measures must be taken to prevent wrong-site, wrong-procedure, and wrong-person surgeries.

• A powerful approach is needed that uses multiple strategies together to ensure that the correct procedure is being performed on the correct site of the correct person.

• It is essential that there is active participation of all the members of the surgical team for success. Effective methods for communication between all the members are also necessary.

• Implementation of standardized and reliable protocol is very effective in achieving patient safety.

Requirements of Universal Protocol:

• Verification process must be conducted before the procedure is commenced.

• Correctly identify and mark the procedure site.

• A time-out is carried out prior to the procedure.

Compliance with the NPSGs 2013 is essential for healthcare organizations in order to avoid severe penalties. Hence, employees are made aware of these goals through training and seminars so that they can comply with them and keep their patients safe.

Perimenopause and Why Topical Bioidentical Hormones May Help You Stay Young and Cancer Free

The onset of menopause can be a difficult and confusing time for a woman. Symptoms can be as vague as weight gain, sleeplessness, memory loss, dry eyes, low libido, depressed mood, or irritability. In fact the cardio protective effects of estrogen are well known to be neutralized with synthetic progestins but not by human progesterone. In fact your body is designed to have many babies over your lifetime and to encounter a veritable flood of natural hormones in a periodic way. I have never heard that having many kids causes cancer, indeed women with no children are at increased risk.

Why pregnant mare urine? It's plentiful and therefore cheaper to produce. Why don't the drug companies just focus on natural human hormones? I don't know all the details but I do know you cannot patent estrogen. Oral hormones also pass first to the liver where they raise the levels of Sex Hormone Binding Globulin (SHBG) which is bad. So many doctors have abandoned HRT altogether, and therefore many women assume that only cancer and death lurk in trying to outfox mother nature. Other physicians myself included agree that synthetic horse urine taken orally may not be a good idea. I would use one word to describe H A Hyaluronic Acid Anti-Aging Cream, Pure Elegance by Organika Health Products of British Columbia. The word for this anti-aging cream is luxurious.

This product was purchased at my local natural products store. This is an excellent anti-aging cream, very luxurious in texture. It can be applied both night and day and can be worn under makeup. Transitioning Through Menopause and Middle-Age With Ease


Transitioning through menopause is challenging. Do we take hormone replacement therapy? Only you can answer that. Do you have body fat and fluid changes, and hunger cravings before your menses? This may be a clue whether hormone therapy will actually benefit you. More breast and ovarian cancers seem to occur during this transition through menopause. Because these cancers are estrogen dependent, I chose not to take hormone replacement therapy.

Starting in peri-menopause, especially if you are working, are you more stressed? Are you hungry all the time? Have you previously noticed that before your menses, that you are retaining more water, and hungry all the time? Peri-menopause and menopause is the time when the progesterone, and testosterone are dropping out of the mix, creating unbalance. Since we have an excess of estrogen, and since cancers like breast cancer and ovarian cancer, (and uterine fibroids) are driven by an excess of estrogen, I think it would be unwise to supplement with hormone replacement therapy.

It is challenging, navigating the transition through menopause, and even middle age. Yes, I used to be addicted to giant chocolate bars. I knew that I was not feeling better when I ate these. I was losing energy, good skin, and retaining more water and fat cells.

Have you noticed that things that you were perfectly happy with before, now become intolerable? I have read that this is the age when women try to rectify the things that they think are unfair. Has communication with your spouse or your children come to heated arguments, with no resolve? Hormone replacement (I believe) will exacerbate the situation. So how do we get back in balance?

Lets start with healthy eating. Yes we may need to fine-tune it a bit. We are all genetic individuals, and we all have different requirements to help our bodies, and cells feel better. What is "healthy" for one may not be healthy for another. Fine tuning our intake of food, can give us immediate results. I was pleasantly surprised that I actually felt good eating something "good for me" rather than feeling the same as I did before. The change in how I felt was dramatic. I could give up the chocolate bars (slowly) and eat the way I knew I should.

I still notice those "monthly" cycles, even though I have no menses. I get more hungry, and I don't digest as well. See if you notice these times too. I promise, if you eat what is good for you, you be able to eliminate the ups and downs of these cycles, making them easier. When the stress and other symptoms subside, we can have less of that "PMS" (out of control) feeling.

I have been able to find my perfect "weight" for my age, without energy loss, and breast cyst recurrence. I have been through the almost dissolution of my marriage, and the animosity of my children. In fact when I worked at a large insurance company, I was sad to notice, that most of the households broke up between the 40's and 60's with most concentrated around the early 50's.

If you have a genetic predisposition to put on belly fat because of your ancestors, you can start to stop it and minimize the "belly". I know because it runs in my family.

I know I can help you with your health, and your relationships, and your personal self-worth. I promise you will not feel hungry or have an upset stomach. You may be able to avoid type II diabetes, and maybe this will even help you avoid the Big C.

Hormones and Quality of Life


As we age our hormones decrease. This can be a problem for many of us. Our body and mind just doesn't seem to work as well without proper hormone levels.

Interview With Michael E. Platt, Author of The Miracle of Bio-Identical Hormones


Today, Tyler R. Tichelaar of Reader Views is pleased to be joined by Michael E. Platt, M.D., author of the newly published 2nd edition of "The Miracle of Bio-identical Hormones."

Michael Platt, M.D. is a bio-identical hormone expert and wellness physician. Unlike most physicians, Dr. Platt believes in being pro-active and treating the causes of illness rather than solely the symptoms. He has successfully helped patients relinquish medications and use natural bio-identical hormones to increase health in his patients. He has devoted thirty-four years to balancing hormones.

Tyler: Welcome, Dr. Platt. To begin, could you give us a definition of "bio-identical hormones" for readers who are not familiar with the term?

Dr. Platt: Bio-identical hormones are molecularly identical to the hormones the body produces; they are readily recognizable to the body and can reproduce the same functions.

Tyler: Dr. Platt, would you tell us a little bit about your medical background and education and how you became interested in bio-identical hormones?

Dr. Platt: I went to medical school at N.Y. Medical College and did my internship and residency in Washington, D.C. at Washington Hospital Center. I am board-certified in Internal Medicine. I started out my career working with various HMO's because in the old days their "reason for being" was preventive medicine. When insurance companies bought them out, preventive medicine was thrown out and it became "managed care." This was their way of saying: what's the least amount of money we can spend and still keep the patient alive? Subsequently, I worked at various medical groups, became a hospitalist, and also took care of a large number of convalescent home patients. The latter ingrained in me a strong appreciation for preventive medicine. Putting everything together--my work in clinics, hospitals, convalescent homes--gave me a strong exposure to traditional medicine. It became clear that medicine practiced this way often ignores the causation of illness. This led me into the world of bio-identical hormone therapy.

Tyler: I understand balancing hormones is still not a common topic in medical school, yet you have devoted most of your life to studying it. Why do you think it hasn't gained more attention?

Dr. Platt: Most doctors appear to be more comfortable with advice provided by drug companies. Since bio-identical hormones are natural and cannot be patented, drug companies have no interest in promoting them for health. As you may or may not be aware, almost everything doctors learn in medical school is based on research done by drug companies. Needless to say, there is no mention of bio-identical hormones. In spite of this, the popularity of Bio-Identical Hormone Therapy (BHT) will continue to grow due to patient demand and successful BHT outcomes with patients.

Tyler: The subtitle to your book is "how I lost my fatigue, hot flashes, ADHD, ADD, fibromyalgia, PMS, osteoporosis, weight, sexual dysfunction, anger migraines." Would you give us an example or a success story detailing how balancing hormones has helped someone with one of these ailments?

Dr. Platt: I approach a patient's problem from the cause; I do not prescribe band-aids. Hormones control every system in the body so it is easy to see there are a lot of medical conditions that can be positively influenced by getting hormones into balance. Throughout my book, patients tell their own stories about being un-well and how hormones have restored them to optimum health. Instead of relating a particular example of a specific patient who was helped, please allow me to approach your question from a different direction.

The majority of patients who come to me with "incurable" diseases often have a condition known as ADHD. ADHD may be one of the most unrecognized and misunderstood of all medical conditions. It is a condition that has been misrepresented by the medical community as many doctors do not actually focus on or understand its underlying cause.

I approach ADHD strictly from a hormonal aspect, since it represents an imbalance of hormones--too much adrenaline, too much insulin, and too little progesterone. It is possible to eliminate this condition, in many cases, within 24 hours. The reason I mention this particular condition is that it is associated with many other conditions such as fibromyalgia, restless leg syndrome, bipolar disorder, rage problems, endometriosis, prostate cancer, and type II diabetes. When you view it from this perspective you can appreciate right away how restoring hormone balance can be life altering.

Tyler: Dr. Platt, you have been described as a physician who is not a favorite of pharmaceutical companies. What do you see are the dangers of large pharmaceutical companies and their relationship with the medical field?

Dr. Platt: The medical system in the U.S. is a multi-trillion dollar a year industry. It thrives on profits it makes from disease and illness, not on wellness. It provides millions of jobs, if you include all the ancillary industries involved with medical care. The medical system also provides a huge source of tax revenue for the government, so there is no incentive to change things. If people want to be healthy, they must become pro-active about their health and not rely on an industry that is not dedicated to their well-being. Drug companies obviously have no interest in a person who is healthy; and drug companies have almost complete control over how most doctors practice medicine.

Tyler: You have expressed frustration that pharmaceutical companies fund much of the research being done, but research is seldom done on natural hormone supplements because no patents are available for these products and patents are where the real money is. Could you explain why patents aren't available for these products, and do you have any suggestions for how this matter can be resolved?

Dr. Platt: It is estimated that prescription drugs are perhaps the leading cause of death in this country. The body cannot recognize synthetic chemicals; synthetic chemicals can create toxicity in the body. The body can recognize natural products and can accept them according to need.

The issue of synthetic vs. natural will slowly be resolved through patient education. Please be aware that although my book addresses some of these issues, it is not intended to be a diatribe against the medical field; I am proud to be a practicing board-certified internist. I include discussion of these issues to outline part of the reason some people are not well.

Tyler: I understand one of your major concerns is about estrogen replacement. Would you elaborate on that issue, please?

Dr. Platt: Estrogen, to my way of thinking, is a toxic hormone. It can cause six different cancers in women, as well as strokes and heart attacks. It causes women to have cramps, PMS, and migraine headaches with their period, and can cause fibroids, endometriosis, fibrocystic disease, gall bladder disease, asthma, lupus, and rheumatoid arthritis. Interestingly, women never stop making estrogen. So why is this hormone being replaced with such regularity in light of the fact that it is such a toxic hormone? The only hormone that has to be replaced is progesterone--a hormone that prevents every downside of estrogen.

Tyler: I understand the issue of estrogen has a personal connection to you in relation to your mother. Would you mind telling our audience a little bit about your mother's illness and how it affected you?

Dr. Platt: My mother died at the age of 61 of breast cancer. She had been estrogen dominant her whole life, developed fibroids, had a hysterectomy and was subsequently placed on Premarin. At the time of her death I was looking toward the causation of illness. It was readily apparent that estrogen was the contributing factor for her problems. However, I put a number of things together and realized the cause of her problems was actually lack of progesterone--even to this day an unknown hormone to most doctors.

My mother had a large abdomen, but no fat on her extremities. The only hormone that can cause this is insulin. Could her increased estrogen and increased insulin be related? Shortly after her death, I had what you might call an epiphany. I remembered that men and women have identical hormones. I suddenly realized I had inherited her low progesterone because my whole life I had been over-producing insulin (daytime fatigue, sleepy when driving, craving sugar, fighting weight, ADD in school, etc.) At this point I started using natural, bio-identical progesterone and my whole life changed course. Needless to say, my mother's illness had a profound effect on what I do today.

Tyler: The second edition of "The Miracle of Bio-identical Hormones" is being released this summer. Why did you feel a need for a second edition?

Dr. Platt: The second edition was primarily the result of my reading about a tragic event that occurred in December, 2006, in Massachusetts. A beautiful four-year old girl died of an overdose of medication she was receiving to treat her "bipolar disorder." Both of her parents were arrested and will stand trial for the child's death.

The child was more than likely ADHD. Her doctor had been treating her with dangerous prescription drugs for two years and apparently had never seen her. The people at her school, as well as the social service department that was following her case, apparently observed she was being overdosed. The pharmacist, as reported, provided refills of the medications, apparently every other day, another example of overdosage.

The question is, who is responsible? The parents? The whole medical/social system? Were the parents even aware their daughter might have ADHD? I approach maladies such as ADHD and bi-polar disorder, from a hormonal aspect. I believe I am able to heal patients such as this little girl with natural bio-identical hormones. As far as I know, I may be the only physician in the world who approaches such challenging conditions with this protocol. Note: It is a documented fact that thousands of children a year die from prescription drug overdoses for a disorder few doctors seem to understand.

So, I felt it was necessary to share my approach to this condition, and I added more about it in the second edition. In addition, I added a chapter about osteoporosis. In my opinion, women's bodies and emotions become brutalized with the use of medications such as Fosamax, Actonel and Binova, all of which actually prevent bone from healing.

Tyler: I visited your website http://www.drplatt.com and saw a questionnaire there to help people determine if they have hormone imbalances. What advice do you have for them if that is the case?

Dr. Platt: The questionnaire on the website at http://www.drplatt.com is answered promptly by the office; the answers provide information that may indicate a need for hormone replacement. With this knowledge, people can visits their own doctors to request the specific tests I talk about that would benefit them. It will also provide them with precise advice on which bio-identical hormones and dosages could be of benefit. For those who prefer, they are welcome to make an appointment to come see me. 90% of my patients are from out of town and out of the country, including physicians and their families.

Tyler: Dr. Platt, your work remains controversial and the pharmaceutical companies remain powerful. Do you think there is hope for change in the near future? Is the future of medicine going to be pro-active and about preventive medicine?

Dr. Platt: My work is certainly controversial because I take people off their drugs and put them on bio-identical hormones. Prescription drugs are the leading cause of death and morbidity--this is the standard of care. As long as drug companies continue to tell doctors how to practice medicine, this will not change. However, I predict there will be a ground swell of people who will rebel against the status quo, and many doctors will give in and grudgingly change the way they practice. Again, not because they embrace it, but because their patients will demand it. I love medicine. It is very rewarding to have a patient say to me, "Doc, in my entire life I have never felt so good." I hear this all the time; traditional doctors never hear this. So yes, people will become proactive about their health and will become aware of the power of preventive medicine.

Tyler: Thank you, Dr. Platt, for being here today. Do you have any last advice you would like to give to our readers?
